Job Description

ProMedica Hospice, formerly Heartland Hospice, is currently searching for a full-time Medical Social Worker to support their hospice patients in the Rockford and surrounding areas.  

The Medical Social Worker is responsible for providing patients and families psychosocial care regarding economic, social, and emotional needs.  This position requires a Masters in Social Work and social LSW or LCSW in IL.  WI licensure is required as well.
